C-spine injury and quadriplegia. Severe itching over his both forearms, which has previously treated as Scabies. The itching persisted. We have given him Vasoline and Topical steroids, but the response was not satisfactory.

But from the history, it's quite common that a patient with scabies was usually overtreated with scabicides that might induce dry skin and itching consequences. Maybe you could try stop the antiscabies treatment and give some emllient. And if that doesn't work in 1-2 week, you should visit a dermatology specialist. Because general skin condition evaluation and close observation on skin lesions at present is critical for an appropriate diagnosis. Only one photo with poor quality is not adequate for that purpose.
